Dr. Mohindra is a Stanford trained Interventional Radiologist who is board certified in Vascular and Interventional Radiology and Diagnostic Radiology by the American Board of Radiology. He is licensed to practice medicine in multiple states, including Indiana and is a member of the American Medical Association (AMA), the Indiana State Medical Association (ISMA), the Society of Interventional Radiology (SIR), and the Outpatient Endovascular and Interventional Society (OEIS).

Nurse Practitioner

Tyler Kelly is a Nurse Practitioner, board-certified by the American Nurses Credentialing Center, and licensed by the state of Indiana. Tyler’s has an ideal background to provide focused, compassionate, insightful, and exceptional care to End Stage Renal Disease patients. In his career he has worked in dialysis units, as an NP in a Nephrology practice, and in multiple capacities within a Vascular Access Center.
